This Privacy Policy explains how Storms Karate, LLC d/b/a Storms Health (“Storms Health,” “we,” “us,” or “our”) collects, uses, and shares information when you visit Apex Health & Performance or complete our health survey.

Information we collect

We may collect contact details such as your name, email address, phone number, mailing address, and preferred contact method. Our health survey may also request information about your goals, lifestyle, movement, sleep, eating habits, and health-related history that you choose to provide.

How we use information

We use information to respond to inquiries, review survey submissions, communicate about coaching services, schedule or deliver requested services, operate and improve our website, and comply with legal obligations. We may send marketing emails or text messages only when you separately choose to provide the applicable consent. Consent to marketing messages is not a condition of purchasing goods or services.

How information is shared

We may share information with service providers that help us operate our business, including form, scheduling, communication, hosting, and technology providers. Mobile information and SMS consent

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Survey and health information

Please provide only information you are comfortable sharing for the purpose of discussing health coaching. This website and survey are not emergency services and are not a substitute for professional medical diagnosis, treatment, or emergency care. If you have a medical emergency, call 911 or seek immediate medical attention.

Your choices

You may ask to review, correct, or delete personal information we hold, subject to applicable legal and operational requirements. You may opt out of marketing emails by using the unsubscribe option in the email. To stop marketing text messages, reply STOP; for help, reply HELP. Message and data rates may apply.

Third-party services

Our survey is hosted by Jotform. We use a HighLevel/LeadConnector chat widget that can present optional SMS communication consent. When you use either third-party service, that provider’s privacy practices may also apply. Please review the provider’s policies before submitting information.

Children’s privacy

This website and its services are not directed to children under 18, and we do not knowingly collect personal information from children under 18 through this site.

We may update this policy from time to time. The updated version will be posted on this page with a revised “Last updated” date.
