These Terms & Conditions govern your use of Apex Health & Performance and its health survey. The site is operated by Storms Karate, LLC d/b/a Storms Health (“Storms Health,” “we,” “us,” or “our”). By using the site, you agree to these Terms and our Privacy Policy.

Informational and coaching purpose

The website, survey, and any coaching-related content are provided for general educational and wellness-coaching purposes. They are not medical care, diagnosis, treatment, or a substitute for advice from a licensed physician or other qualified healthcare professional. Always seek the advice of a qualified healthcare provider about any medical condition, treatment, exercise program, diet, or health concern.

Emergencies

Do not use this website or survey for emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate medical attention.

No guarantee of results

Health and performance outcomes depend on many individual factors. We do not guarantee any particular result, including changes in weight, strength, energy, appearance, health status, or performance.

Survey submissions

You agree that information you submit is accurate to the best of your knowledge. Submission of a survey does not create a client relationship, guarantee enrollment, or obligate either party to purchase or provide services.

Text messaging program

Storms Health may send marketing text messages about special offers, discounts, coaching services, and service updates only after you separately choose to opt in through the chat widget. We may respond to an inquiry using the contact information you provide. Consent to marketing messages is not a condition of purchasing goods or services.

You can cancel the SMS service at any time by replying STOP to the number from which you receive a message. After you send STOP, we will send a confirmation that you have been unsubscribed, and you will no longer receive SMS messages from us unless you sign up again.
